Reports taken by city police or a sheriff
Reports taken by city police and county sheriffs also go into the same state database for purchase; if you lack the date, last name or county, NDDOT says to contact the law enforcement agency that investigated the crash.
The form
Officers in North Dakota write crash reports on the North Dakota crash report (electronic report submitted by the investigating officer under NDCC 39-08-10). Most boxes on it hold numeric codes for things like contributing factors, injuries and road conditions.
After you have the report
North Dakota gives you six years to file a personal injury lawsuit (N.D. Cent. Code § 28-01-16(5)). Every North Dakota deadline and fault rule.
North Dakota is a no-fault state: your own personal injury protection (PIP) pays first, with at least $30,000 required, and you can sue the other driver only past the state's threshold. Bills for treatment that are sent in more than 180 days after the treatment date do not have to be paid by you or by the no-fault insurer, so make sure your providers bill promptly.

The name of the motor vehicle insurance policy carrier and the policy number of the driver ... must be furnished to the law enforcement officer investigating the accident. If the driver does not have the required information concerning insurance to furnish to the investigating law enforcement officer, then within five days of the accident the driver shall supply that information to the driver's license division in the form the division requires.

The driver of a vehicle involved in an accident resulting in injury to or death of any individual, or property damage to an apparent extent of at least four thousand dollars, shall immediately give notice of the accident to the local police department if the accident occurs within a municipality, otherwise to the office of the county sheriff or the state highway patrol. A driver who violates this section must be assessed a fine of fifty dollars.
