Reports taken by city police or a sheriff
Local agencies' reports are in the same BuyCrash repository. The City of Indianapolis says: "You can find accident reports online at BuyCrash.com. An accident report costs $12. You must provide the date, time, and location of the acc[ident]" (search-result excerpt of indy.gov).
The form
Officers in Indiana write crash reports on the Indiana Officer's Standard Crash Report. Most boxes on it hold numeric codes for things like contributing factors, injuries and road conditions.

The BMV Driver's Manual says a driver must stay, give name, address and registration, and provide assistance, and that after a crash the BMV may ask for proof of financial responsibility: "After an accident and upon request from the BMV, you will be required to provide proof of financial responsibility to the BMV." No driver-filed crash report was found in it.

"As soon as possible, the driver should make sure law enforcement (local police, sheriff, or state police) are notified of the accident. If the collision was with an unattended vehicle or other property, the driver must stop and remain at the scene of the accident, take reasonable steps to notify the owner of the damaged property, and if the owner cannot be located, call a law enforcement agency in order to provide information."
