Reports taken by city police or a sheriff
Reports taken by city police and county agencies come from that agency's records unit; Las Vegas Metropolitan Police charges $13.00 per Traffic Collision Report (effective July 1, 2026), takes online, in-person or mail requests with photo ID, and says to allow up to 10 business days after the report is filed.
The form
Officers in Nevada write crash reports on the Nevada police crash report (officer's report; form prescribed by the Department of Public Safety under NRS 484E.120); driver self-report is Form SR-1 'Report of Traffic Crash'. Most boxes on it hold numeric codes for things like contributing factors, injuries and road conditions.

the driver of a vehicle which is in any manner involved in a crash on a highway or on premises to which the public has access, if the crash results in bodily injury to or the death of any person or total damage to any vehicle or item of property to an apparent extent of $750 or more, shall, within 10 days after the crash, forward a written or electronic report of the crash to the Department. | DMV SR-1: Pursuant to NRS 484E.070, this SR-1 report needs to be completed within 10 days after a crash that occurred in the State of Nevada and was NOT investigated at the scene by law enforcement.

If no police officer is present, the driver of any vehicle involved in a crash described in subsection 1, after fulfilling all other requirements of subsection 1 and NRS 484E.010, insofar as possible on his or her part to be performed, shall, if the crash resulted in injury to or death of any person or occurred on a highway, forthwith report the crash to the nearest office of a police authority or of the Nevada Highway Patrol and submit thereto the information specified in subsection 1.
